Abstract: Lack of adequate methods of an "on demand" multiple ligand loading within purified exosome lumen and off the shelf administration of lyophilized formulations against treatment of health problems such as tumors or autoimmune /autoinflammatory diseases hampered translational advances of exosome-based therapies in the clinic. Herein, we describe a fast and efficient biological cargo loading method that warrants high-yield co-entrapment of a protein antigen, single or double stranded nucleic acid-based immunoadjuvants, immunosuppressive DNA molecules and lipidic antigen or a superparamagnetic FeO nanoparticle following purification of exosomes from biological fluids. The lyophilization method retained the exosome membrane integrity and protected labile ligands from premature digestion, enhanced internalization yield by target cells, and amplified in vivo immunomodulatory and adjuvant actions. The immunotherapeutic utility of multiple ligand encapsulating exosomes were examined either as a cancer vaccine or as a preventive agents in mice. I will discuss our recent data on the optimization and application of this technology aiming to treat/downregulate several complications.
